Privacy Notice

The Evening Sanctuary (as part of Mosaic Clubhouse) needs to collect data about you, how you use the service and any work that we do on your behalf. This is important to make sure that we provide a good service and to demonstrate our work to our commissioners and funders.  
We are committed to being clear about our data collection and to meeting our legal obligations.
We rely on the following legal basis for our data collection: 6(1)(f ) – Necessary for the purposes of legitimate interests pursued by the controller or a third party, except where such interests are overridden by the interests, rights or freedoms of the data subject; 9(2)(h) – Processing is necessary for the purposes of preventative or occupational medicine, for assessing the working capacity of the employee, medical diagnosis, the provision of health or social care or treatment or management of health or social care systems and services on the basis of Union or Member State law or a contract with a health professional.

What information does the Evening Sanctuary collect and process about you?

• Personal data from your referral form, such as name, address, telephone number, date of birth, gender, support needs, risk assessment, etc.
• Sensitive data such as information about your ethnic origin and your mental health.
• Your activity at the Evening Sanctuary such as attendance, incident/accidents, participation in groups and the monitoring form filled in by you at the end of the session.
• Staff observations about you recorded each time you attend.
• We may also record other information about your benefits, housing or right to work when relevant.

Evening Sanctuary (run by Mosaic Clubhouse) collects this information in various ways: from application forms; from signing in sheets; other forms you complete including the daily monitoring form , from communication sent out to you & from observations in the Evening Sanctuary. In some cases we may collect personal information such as risk assessment from other services.

Why does the Evening Sanctuary process your personal data?

Evening Sanctuary, as part of Mosaic Clubhouse, has to process data to carry out its work and provide a service to you. In some cases, we need to process data to meet our legal obligations; in other cases the organisation has a legitimate interest in processing personal data. Processing your data allows us to:  

• Process referrals to the Sanctuary/Mosaic.
• Keep accurate and up-to-date records and contact details.
• Keep records of the support we provide.
• Report back to our commissioners & funders.
• Evaluate our work.

Who has access to your data?

Staff working at the Sanctuary/Mosaic.
We will never share your personal information with anyone else without asking you first unless it is an emergency, a safeguarding issue or if we have to do this by law.

How does the Evening Sanctuary protect your personal data?

We take the security of your data seriously. We have policies and controls in place to ensure that your data is protected. Paper files are stored in locked cabinets. Electronic information is password protected and monitored. If you would like more information about this, please ask a support worker.

For how long does Evening Sanctuary keep data?

We keep all information for as long as you are an active user of the Sanctuary. After you stop using the Evening Sanctuary we will only keep the information for 10 years.

  Your rights

• You can access and get a copy of your data;
• You can ask  the Sanctuary to change incorrect or incomplete data;

What if you do not want to provide personal data?

We need to collect data about members se we can provide a safe service to our members and to meet our contractual obligations to our funders and commissioners. If you do not provide your information we will not be able to do so.
